Whether you need your carer to handle all of your chores or would simply benefit from a little extra help, they’ll be able to assist with cleaning, vacuuming, laundry, dusting and so much more - ensuring you can always enjoy a fresh and cosy home

Medication assistance 

Every live-in carer at Silver Crescent Care is trained to safely help with medication. This includes collecting a prescription on your behalf, helping to organise medication for the right days and times, and assisting with the administration.

Our carers are all selectively recruited for their kind and compassionate approach to their roles, which means they’re great at being a fun companion, a helping hand, a listening ear and a shoulder to lean on

We provide supported living that helps people to gain more control over their lives, encouraging them to be confident and happy to make their own choices and decisions.


 

What is supported living?

Supported living (or supportive living) is a service that helps people with a range of support needs retain their independence whilst still having that extra physical and emotional support needed to lead completely fulfilled lives in their own homes. 

Our support 

We support adults across the UK living with one or a combination of conditions, which may include learning disabilities, autism, Physical disabilities, or mental health needs. We offer both core and 1:1 support, all of which is designed around the individual needs and wishes of each person by our dedicated and specialist teams.

We weave quality into everything we do. Our teams have specialist training, allowing them to capably and confidently deliver the highest quality service for the people we support.

What is moving into supported living like?

A move into supported living can be the first time someone has left their family home or taken a step towards greater independence, and so we're here to provide friendly encouragement and emotional support to make the transition as smooth, positive and rewarding as possible.

When you choose supported living, you’re choosing to live independently, safe and secure behind your own front door. There’s peace of mind in knowing the Silver Crescent Care team is on-hand, providing tailored support that helps people to lead more fulfilling lives, surrounded by like-minded people. We also provide support to families to ensure that the transition is not only smooth for the individual but also their families.

What is supported living like with Silver Crescent Care?

Supported living is designed to help people live happy lives filled with ambition. Whether it's learning a new life skill such as cooking or money management, having the confidence to take up a new hobby or make new friends, or taking the first step into volunteering or employment, we're here to support and encourage each goal, however big or small. 

We know that it's really important to have the right support network beside you, so we make sure to carefully match our support team with each person, considering our team's skills, knowledge and specialism, along with personality traits, hobbies and interests to make sure that we nurture friendly and rewarding relationships. 

 

How we create our supported living plans? 

Our support plans are only created once we've really got to know each person and built a clear understanding of their history, their current situation and their future goals. We have in-depth conversations with the person, their family and any relevant care professionals to make sure that we have all the necessary information and have taken the time to hear each person's views and wishes before we provide our care. Once a supported living support plan is in place, we regularly review progress, recognising and celebrating every achievement and continuing to encourage each person to set new goals to achieve even more. 


 

How our process works

Step 1 

To move to a supported living service you will need to have funding in place. Contact your local council or GP who will put you in touch with a social worker to help get your support agreed. 

Step 2 

Once your local council has agreed your support, you can then get in touch with Silver Crescent Care. One of our Referral team will then get in touch with you.

Step 3 

We'll arrange a visit in person to get to know you a little better. We'll talk about your current situation, support need and also your short, medium, and long term ambitions. 

Step 4

We'll arrange visits to our available apartments or shared houses in your chosen location so you can have a look around and ask our team any questions you might have.

Step 5

We'll create you a support and funding proposal and send it to your local council for approval - keeping you up to date throughout the process.

Step 6 

Once funding is approved we can set a move in date, and will be on hand to support you through the whole transition process to make the move as smooth as possible.

Categories of supported accommodation

There are 3 categories of supported accommodation that we can arrange. 

24 hr supported housing

This accommodation is designed for the sole use of the service user placed there. 

 

Self-contained properties with floating support in the day or overnight staff support. 

 

 

Shared housing

Shared housing is suitable for everyone and offers lots of benefits for leading a fulfilling and more independent life, especially for someone who is looking for a family-home feel.
